Për të zhvlerësuar një plan terrorist, një agjent i FBI-së i nënshtrohet një operacioni të transplantit të fytyrës dhe merr identitetin e një krimineli gjenial. Plani dështon kur krimineli zgjohet para kohe dhe kërkon hakmarrje. Në këtë kryevepër të zhanrit aksion, agjenti Sean Archer detyrohet të “veshë” fytyrën e armikut të tij të betuar, Castor Troy, për të zbuluar vendndodhjen e një bombe biologjike. Por loja kthehet në një makth kur Troy merr fytyrën e Archer-it, duke vjedhur jetën, familjen dhe identitetin e tij.
